ความสัมพันธ์ระหว่างองค์ประกอบของร่างกายระดับน้ำตาลในเลือดกับค่าสมรรถภาพปอดในผู้ป่วยเบาหวานชนิดที่สอง

| dc.description | Type II diabetes mellitus caused by lost of the insulin- producing of B-cells that are attempted to the body, to compensate for the insulin resistance of tissues. In addition, insulin resistance is associated with abdominal fat. Several patients were obese. The accumulating fat in whole body affected to arterial defects, that showed basal lamina thickening, including pulmonary capillaries. High blood glucose levels associated with increased levels of systemic inflammatory with microangiopathy, that cause of the impairment of gas exchange and abnormal lung functions. The aims of this study, to examined the pulmonary function of type II diabetes mellites and evaluate the correlation between body compositions, blood sugar level and pulmonary function. Twenty-eight type II diabetes mellitus patients with age between 40-65 years old were enrolled. Datas were collected with a structured questionnaire, such as weight, height, body mass index (BMI), waist-hip circumference, blood sugar level and duration of diabetes. Then, subjects were measured pulmonary function by spirometer. The results showed a significantly decrease of FVC, FEV1 and PEF, compared with predicted value. FVC was a positive correlated with waist-hip ratio. Neither blood sugar level nor duration of disease were correlated with pulmonary function. In conclusion, type ll diabetes patients have reduced lung function in restrictive pattern. Durations of diabase and blood sugar level were not affected to pulmonary function. | |
| dc.description.abstract | โรคเบาหวานชนิดที่ 2 เกิดจากการสูญเสียหน้าที่ของ β-cells ของตับอ่อน ที่พยายามผลิตอินซูลินให้เพียงพอกับความต้องการของร่างกายเพื่อทดแทนการดื้อต่ออินซูลินของเนื้อเยื่อต่างๆ ภาวะการดื้อต่ออินซูลินยังมีความสัมพันธ์กับไขมันในช่องท้อง ผู้ป่วยส่วนใหญ่จะมีรูปร่างอ้วน มีไขมันสะสมในส่วนต่างๆ ของร่างกาย ทำให้เกิดพยาธิสภาพของหลอดเลือด โดยจะส่งผลให้ผนังหลอดเลือดหนาตัวขึ้น รวมถึงหลอดขนาดเล็กภายในปอด ระดับน้ำตาลในเลือดที่สูงจะทำให้เกิดการเสื่อมและการอักเสบของผนังหลอดเลือด และเนื้อเยื่อปอดหนาตัว ซึ่งความผิดปกติที่เกิดขึ้นบริเวณเส้นเลือดในปอดและเนื้อเยื่อปอดนี้ จึงเป็นสาเหตุหลักที่ขัดขวางการแลกเปลี่ยนก๊าชภายในปอดส่งผลให้สมรรถภาพปอดผิดปกติตามมา การศึกษานี้มีวัตถุประสงค์ เพื่อศึกษาค่าสมรรถภาพปอดในผู้ป่วยเบาหวานชนิดที่ 2 และหาความสัมพันธ์ของสมรรถภาพปอดกับองค์ประกอบของร่างกาย และระดับน้ำตาลในเลือด โดยทำการศึกษาในผู้ป่วยเบาหวานชนิดที่ 2 ที่มีอายุมากกว่าหรือเท่ากับ 40 ปี จำนวน 28 คน ทำการบันทึกข้อมูลสุขภาพเบื้องต้น ได้แก่ อายุ น้ำหนัก ส่วนสูง ดัชนีมวลกาย (BMI) เส้นรอบวงเอว และรอบสะโพก เปอร์เซ็นต์ไขมันในร่างกาย ระดับน้ำตาลในเลือด และระยะเวลาในการเป็นโรคเบาหวาน ประเมินสมรรถภาพปอดโดยใช้เครื่องสไปโรมิเตอร์ ผลการศึกษาพบว่า ผู้ป่วยเบาหวานชนิดที่ 2 มีสมรรถภาพปอดลดลงอย่างมีนัยสำคัญทางสถิติ เมื่อเปรียบเทียบกับค่าคาดคะเน โดยเฉพาะค่า FVC, FEV1 และ PEF และด่า FVC มีความสัมพันธ์เชิงบวกกับสัดส่วนรอบเอวต่อรอบสะโพก ระดับน้ำตาลในเลือดและระยะเวลาในการเป็นโรคเบาหวานไม่มีความสัมพันธ์กับค่าสมรรถภาพปอด ผู้ป่วยโรคเบาหวานชนิดที่ 2 มีสมรรถภาพปอดที่ลดลงแบบปอดถูกจำกัด (restrictive lung) ) ระยะเวลาในการเป็นโรคเบาหวานที่ยาวนาน ส่งผลให้มีการสะสมไขมันในช่องท้อง ทำให้สัดส่วนรอบเอวต่อรอบสะโพกเพิ่มขึ้น ผู้ป่วยโรคเบาหวานชนิดที่ 2 จึงมีลักษณะอ้วนลงพุง | |
